FIELD FINDING: Unsecured Pressurized Air Hose

BEFORE:
A pressurized air hose was connected to an air nozzle, but the locking mechanism was not pinned, and there was no anti-whip restraint installed.
RISK:
If the hose were to become disconnected under pressure, it could whip uncontrollably, creating a serious struck-by hazard and risking injury to nearby personnel.
AFTER:
Following our observation, the site team installed locking pins and anti-whip restraints, immediately eliminating the hazard.

FIELD FINDING: Incomplete Scaffolding Protection

BEFORE:
Scaffolding for this outage was found with a green tag (indicating it was safe to use), but it was missing the required gate at the top of the access ladder.
RISK:
Without a ladder gate, the platform presents an unprotected fall hazard. This condition requires either fall protection or a downgrade to a caution tag until corrected, for the duration of the outage project.
AFTER:
Our safety professionals worked directly with the Scaffolding Supervisor, providing two options:
✅ Install the ladder gate immediately
✅ Or re-tag the scaffold to caution status and enforce fall protection PPE until corrected.
The team chose to install the gate the same day, restoring safe access.
